Hayden Panettiere’s cause of death has been revealed, with authorities determining that the actress died from the toxic effects of fentanyl and several other substances. Her death has been officially ruled an accident.
The Heroes and Nashville star died on August 16, 2026, at the age of 36, after she was found unresponsive at an apartment in Greenville, South Carolina. Emergency responders attempted to revive her, but she was pronounced dead at the scene.
Hayden Panettiere’s Official Cause of Death
According to the Greenville County Coroner’s Office, Panettiere’s cause of death was attributed to the toxic effects of multiple substances.
The coroner identified the substances as:
- Fentanyl
- 4-ANPP
- Alprazolam
- Methocarbamol
- Quetiapine
The coroner classified the manner of death as accidental.
Fentanyl is a powerful synthetic opioid. The other substances listed in the report include alprazolam, commonly known by the brand name Xanax; methocarbamol, a prescription muscle relaxant; and quetiapine, a prescription medication.
Hayden Panettiere Was Found Unresponsive in Greenville
Panettiere was found unresponsive at an apartment in Greenville on August 16. Emergency personnel responded to the scene and attempted advanced resuscitation for more than 40 minutes, according to The Associated Press. She was ultimately pronounced dead.
Police previously said their preliminary investigation had found no signs of foul play or suspicious circumstances.
What Happened Before Her Death?
Panettiere had publicly discussed her struggles with alcohol and drug addiction throughout her life. In her 2026 memoir, This Is Me: A Reckoning, she wrote about addiction, recovery and the pressures she experienced while growing up in Hollywood.
Following her death, authorities investigated the circumstances surrounding the case. The Drug Enforcement Administration also joined the investigation alongside Greenville police and the coroner’s office.
The latest coroner's findings now provide the official cause and manner of death.

Panettiere began acting as a child and went on to become one of television's recognizable young stars.
She became internationally known for playing Claire Bennet in NBC’s hit superhero series Heroes. She later starred as Juliette Barnes in the ABC musical drama Nashville, a role that earned her a Golden Globe nomination.
Her film credits included Remember the Titans, Ice Princess, Scream 4 and Scream VI.
